Understanding the Bruce Test Calculations

The Bruce Protocol is a standard diagnostic test used by clinicians and cardiologists to measure cardiorespiratory fitness (VO2 Max). As the test progresses through set 3-minute stages, speed and incline increase. The calculator uses regression models to translate total test time into estimated VO2 Max. Peak METs reached represents functional metabolic capacity, calculated by applying the ACSM walking/running equations to the speed and grade of your final completed stage. Higher MET scores indicate higher aerobic conditioning and lower risk of cardiovascular disease.

The Bruce Protocol is highly strenuous because the incline starts at a steep 10% and rises up to 22%. It is designed to push the cardiorespiratory system to its limits, which is why it is often monitored by healthcare professionals using an EKG/ECG machine.

The test was created in 1963 by cardiologist Robert A. Bruce. In clinical medicine, stopping early (prior to Stage 3 or under 9 minutes) is often used as a prognostic marker for myocardial ischemia or other underlying heart conditions.

Male Equation:
VO2 Max = 14.8 - (1.379 × Time) + (0.451 × Time²) - (0.012 × Time³)

Female Equation:
VO2 Max = (4.38 × Time) - 3.9

Active/Athletic Equation:
VO2 Max = 3.778 × Time + 0.19

ACSM Walking Equation for METs:
VO2 (ml/kg/min) = (0.1 × Speed in m/min) + (1.8 × Speed in m/min × Incline Grade) + 3.5
METs = VO2 ÷ 3.5
Example

A male who stops the test at 10 minutes and 15 seconds (10.25 minutes) will have a VO2 Max calculated as: 14.8 - (1.379 × 10.25) + (0.451 × 10.25²) - (0.012 × 10.25³) = 35.1 ml/kg/min. For a 35-year-old male, this is classified as a 'Fair' fitness rating, equivalent to approximately 10.1 Peak METs.
